Overview
Reporting to the Manager, Social Care, the Position Holder will be responsible for the strategic development of portfolios to advance social-health integration and enhance community care delivery. This includes managing collaborations with partners and within NUHS to support new policies and initiatives, strengthening NUHS’ role as the population health manager for the western region.
Job Responsibilities
- Develop strategies and work plans to enhance NUHS’ role in new health and social policies, promoting social-health integration.
- Identify opportunities and engage regularly with agency/community partners and internal stakeholders to drive shared successes.
- Oversee end-to-end project management of assigned collaborations or initiatives.
- Stay updated on developments within the health and social landscape.
- Perform secretariat duties to facilitate progress and discussions in assigned portfolios.
Requirements
- Bachelor's Degree.
- 4-6 years of relevant experience, including 1-2 years in a leadership role.
- Experience in planning, strategic partnerships, and portfolio or product management is advantageous.
- Proven skills in project management and stakeholder collaboration.
- Proactive attitude towards portfolio growth.
- Strong communication, interpersonal, and organizational skills.
- Ability to multi-task and work independently in a dynamic environment.
