Primary Responsibilities and Duties (80%)

Patient Care

  • Performs pre-admission screening for patients and caregivers.
  • Collaborates with nurses to manage and provide holistic care to patients at home, including monitoring general condition and progress.
  • Identifies and reports deviations from normal, care gaps, or issues to the team nurse lead or clinicians.
  • Handles inquiries and provides regular updates on care plans with patients and caregivers.
  • Participates in interdisciplinary team meetings and contributes to patient care plans.
  • Documents patient care and maintains accurate records of progress and treatment plans.
  • Coordinates with community resources and healthcare providers for continuity of care and referrals.
  • Develops care plans based on assessed needs.

Telehealth Monitoring

  • Monitors vital signs and escalates red flags to team nurses/physicians.
  • Provides basic patient education.
  • Adheres to telehealth monitoring protocols using the Telehealth system.
  • Provides lifestyle and health coaching, including diet, nutrition, and basic exercise guidance.

Secondary Responsibilities and Duties (20%)

  • Ensures availability of necessary equipment and supplies, coordinating with vendors as needed.
  • Participates in quality improvement activities and risk management projects.
  • Performs other ad-hoc duties as assigned.

Job Specification/Requirements

  • Diploma in health-related courses preferred.
  • Fluent in English; ability to speak local dialects is an advantage.
  • Good computer literacy and knowledge of PC software.
  • Flexible to do shift work, including evenings, weekends, and public holidays.
  • Capable of working independently in a fast-paced environment.
  • Service-oriented, pleasant, cheerful, with strong interpersonal and communication skills.
  • Team player with some leadership qualities.
  • Initiative, committed, with integrity and teamwork skills.
  • Knowledge of hospital operations or healthcare experience is an advantage.
