Singapore Stroke Support Station Full time

Stroke Support Station (S3) is a non‐profit organisation in Singapore dedicated to enhancing the quality of life for stroke survivors and their caregivers. S3 provides a range of programmes focused on rehabilitation, empowerment, and community reintegration. Through specialised activities, wellness programmes, and caregiver support, S3 aims to foster physical, emotional and social well‐being. The organisation also advocates for stroke awareness and collaborates with health‐care professionals, volunteers and partners to offer holistic, person‐centred care for the stroke recovery journey. Responsibilities Conduct accurate and holistic speech and language specific assessment for clients during admission, periodic care plan reviews and discharge; Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making; Formulate person‐centred and SMART goals in collaboration with clients and relevant stakeholders; Design plans and conduct speech and language interventions for clients in a person‐centred manner, that is also evidence‐based; Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines; Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders; Formulate discharge and continuity of care plans based on clients' performance in consultation with relevant stakeholders; Mentor and supervise therapists and support staff for professional development and day‐to‐day job scope; Identify safety risks and issues, recommend preventive and corrective care measures; Conduct in‐house training for staff, caregivers and volunteers; Engage in continuing education to further skills and knowledge in the field of practice. Qualifications Degree or Master in Speech Therapy and fully registered with AHPC; Able to converse in English and other languages due to the diverse profiles of our clients; Minimum of 5 years experience as a Speech Therapist to be considered for a Senior role; Dynamic and proactive personality with good public‐relations and communication skills; Strong management, analytical and problem‐solving skills.
