Job Title: Frontline Service Coordinator

About the Role:

As a Frontline Service Coordinator, you will play a crucial role in delivering exceptional customer service to our clients. You will be responsible for handling incoming referrals, assessing and directing them to appropriate services, and ensuring timely processing.

You will work closely with social workers to conduct financial assessments and collaborate on case management. Your excellent communication skills and problem-solving abilities will enable you to navigate complex situations and provide effective support to our clients.

Key Responsibilities:

  • Handle incoming referrals from the Agency for Integrated Care (AIC), hospitals, and the general public
  • Assess and direct referrals to appropriate services or case officers
  • Collaborate with social workers on case management and financial assessments
  • Maintain accurate records and update referral statuses in our systems

Requirements:

To succeed in this role, you will need:

  • A Diploma in any discipline
  • Experience in health care or community care settings is preferred
  • Strong MSOffice 365 skills
  • Effective communication and interpersonal skills
  • Problem-solving skills
