Job Opportunity: Medical Social Worker Associate

The role of a Medical Social Worker Associate is pivotal in ensuring the well-being of our clients. As part of our dedicated team, you will play a crucial role in facilitating admissions to centre-based services, assessing patients' and families' psycho-social and financial status, and providing accurate information on policies, procedures, and services.

Key Responsibilities:

  • You will report directly to the Centre Manager and be involved in pre-admission work, including liaison with external agencies and internal teams.
  • Assess and evaluate patients' and families' psycho-social and financial status, and assist them in applying for appropriate financial assistance schemes when needed.
  • Work closely with team members in the development of Individual Care Plans and provide accurate information to clients on policies, procedures, and services.
  • Resolve client feedback and complaints in a timely and appropriate manner, working closely with other managers and medical social workers to ensure that all MOH service requirements are met.

Requirements:

  • A Diploma in Social Work or other relevant fields is desirable.
  • 5-days work week with rotational weekends is required.
  • Minimum 2 years of experience in a similar role in a daycare centre, preferably with experience working with MOH or other relevant stakeholders in the healthcare industry is necessary.

Preferred Skills:

  • Experience with IRMS, ILTC Portal, and other relevant IT systems is preferred.
  • Hands-on experience working with the elderly in the community is an asset.
  • Proficient in Microsoft Office and comfortable with IT is essential.
  • Ability to communicate effectively with the elderly is vital.
