• \tAttractive bonus package• \tDynamic working environment• \tOffice hourResponsibilities:• \tConduct pre-admission screening for patients and caregivers.• \tSupport nurses in providing holistic care, including monitoring patient condition and progress.• \tIdentify and report care gaps, issues, or abnormal findings to the clinical team.• \tCommunicate regularly with patients and caregivers, providing updates and handling enquiries.• \tParticipate in interdisciplinary team meetings and contribute to care planning.• \tMaintain accurate documentation of patient care and progress.• \tCoordinate with community partners and healthcare providers for continuity of care and referrals.• \tAssist in developing care plans based on patient needs.• \tMonitor patients' vital signs and escalate red flags to nurses/physicians.• \tProvide basic patient education, lifestyle coaching, and health advice (e.g. diet, exercise, nutrition).• \tEnsure adherence to telehealth monitoring protocols.• \tEnsure availability of equipment and supplies, coordinating with vendors as required.• \tParticipate in quality improvement, risk management, and departmental projects.• \tCarry out other ad-hoc duties as assigned. Requirements:• \tDiploma in a health-related discipline preferred.• \tPrior healthcare or hospital experience is an advantage.• \tGood computer literacy.• \tService-oriented, proactive, and adaptable in a fast-paced environment.
