Job Title: Healthcare Professional

We are seeking an experienced healthcare professional to join our team at a leading community healthcare provider. This is an excellent opportunity for individuals who want to make a meaningful difference in people's lives by providing quality patient care and education.

Key Responsibilities:

  • Provide nursing services at patients' residences, including changing nasogastric feeding tubes, urinary catheters, and dressings.
  • Monitor and report patients' conditions and progress to senior members of the healthcare team.
  • Educate patients and family members on their health conditions and promote holistic care.
  • Assist in executing discharge plans and coordinating post-discharge care for patients.
  • Follow up on phone reviews and home visits to ensure necessary services are in place.
  • Arrange appointments with clients and caregivers for new referrals.
  • Conduct and document nursing assessments as appropriate.
  • Raise individual care plans based on identified assessment needs.
  • Ensure safety and infection control measures are practiced during home visits.
  • Issue receipts and receive payment from clients/caregivers.
  • Carry out administrative work as directed by supervisors.

Required Skills and Qualifications:

  • Diploma or Degree in Nursing
  • At least 2-3 years' experience in providing inpatient nursing care
  • Registered with the Singapore Nursing Board
  • Possess Basic Cardiac Life Support (BCLS) + AED certification
