Nurse Advocate

Job Description:
We are seeking a dedicated and skilled nursing care professional to join our team. As a key member of the healthcare team, you will be responsible for providing high-quality patient care services in a safe, competent, and individualized manner.
You will allocate equipment as necessary to ensure the adequacy of resources to meet care requirements. Effective communication with patients, families, and caregivers is essential for delivering seamless patient care.
The successful candidate will coordinate patient care through advising physicians, provide direct patient care, evaluate patients' conditions, and serve as liaisons between families and physicians.
The multidisciplinary team meetings will be facilitated by you to ensure smooth delivery of patient care and services. You will maintain proper documentation of patients' condition and accurate collation of data. Monitor, reassess, and evaluate patient's condition in implementing care plans.
Nursing care such as medication administration and wound dressing will be performed as required. You will follow up to ensure continuity of patient care and treatment plans.
The Singapore Nursing Board's Standards of Practice for Nurses and Midwives will be enforced by you at all times. High standards of housekeeping and workplace organization must be maintained.
Initiate and drive continuous improvements in own work area on regular basis. You will fulfill role of buddy by mentoring junior staff for performance excellence and career development.
Requirements include at least a Diploma in Nursing from a local Polytechnic and registration with the Singapore Nursing Board. Post-registration experience in a hospital or hospice setting is preferred.
Able to perform under pressure and have good interpersonal and communication skills.
