Responsibilities:

  • Conduct thorough psychosocial assessments for patients and their caregivers.
  • Develop and implement intervention plans in collaboration with healthcare teams.
  • Work closely with multidisciplinary teams to ensure high-quality, safe, and cost-efficient patient care.

Requirements:

  • Hold a degree in Social Work and preferably a Postgraduate Diploma in Social Work or Management/Administration.
  • Be registered as a social worker with the Social Work Accreditation Board (SWAB).
  • Demonstrate ability to work independently and as part of a team.
  • Possess excellent interpersonal and communication skills.
