Job Title: Nurse Counsellor

Job Summary:

NORTHEAST HEALTH INTERNATIONAL PTE. LTD. is seeking a highly skilled Nurse Counsellor to join our team. As a Nurse Counsellor, you will play a critical role in providing high-quality patient care and education on chronic disease management.

Key Responsibilities:

  • Provide comprehensive nurse counselling to patients with chronic diseases, including education on disease management, health promotion, and lifestyle changes.
  • Conduct diabetic foot screenings, foot care education, and diabetic eye screenings.
  • Educate patients on the use of medical devices, such as blood pressure monitors, blood glucometers, and inhalers.
  • Track, document, and review patient progress in chronic disease management and lifestyle changes.
  • Identify patients with poorly controlled clinical indicators and work with them to develop individualized health plans.
  • Deliver direct patient care according to established standards and protocols.
  • Collaborate with Primary Care Coordinators (PCCs) in care coordination and care transition.
  • Provide clinical inputs to various clinics in the network.
  • Review and revise nursing guidelines, standards, and procedures related to chronic disease management.
  • Maintain confidentiality of patient medical records.
  • Participate in continuing nursing education to maintain currency of practice and contribute to quality improvement initiatives.

Requirements:

  • Registered nurse with the Singapore Nursing Board.
  • Currently practicing as a nurse or case manager.
  • At least 5 years of clinical nursing experience as a staff nurse in hospital and/or primary care.
  • Advanced Diploma in Nursing with specialization in Chronic Disease Management.
  • Able to travel around from clinic to clinic.
