Job Title:
Forward-thinking Clinical Pharmacy Leader

Job Summary:
We are seeking a seasoned Clinical Pharmacy expert to spearhead innovative patient care initiatives in our Hospital-at-Home program. As a forward-thinking leader, you will leverage your expertise in pharmaceutical care and medication management to drive transformational change.

Key Responsibilities:
  • Attend daily interdisciplinary rounds to collaborate closely with physicians, nurses, and other healthcare professionals to develop and implement patient care plans in a home-based hospital setting.
  • Conduct daily virtual ward rounds with patients, driving innovation in pharmacological care and ensuring optimal patient outcomes.
  • Utilize extensive knowledge of inpatient care to manage complex cases across a wide range of specialties.
  • Innovate and optimize medication regimens, enhancing patient adherence and minimizing adverse effects, while contributing to the transformation of clinical care models.
  • Provide expert consultation on medication management, ensuring the highest standards of safety and efficacy.
  • Engage in patient education, empowering patients with the knowledge they need to manage their therapies effectively at home.
  • Contribute to clinical governance, strategic planning, and the development and implementation of protocols and guidelines that will shape the future of hospital-at-home care.

Required Skills and Qualifications:
  • Possess MSc (Clinical Pharmacy) or Pharm D or Board Certification.
  • Completion of a Collaborative Prescribing Course within the last 2 years.
  • A minimum of 5 years of clinical experience (ideally in inpatient care).
  • Registered with the Singapore Pharmacy Council.
  • Extensive and current knowledge of pharmaceuticals, clinical practice guidelines and protocols.
  • Strong leadership qualities.
  • Good supervisory, negotiation, and managerial skills.
  • Excellent clinical pharmacy and evidence-based medicine knowledge and skills.
  • Excellent clinical problem-solving, judgment, and decision-making skills.
  • Excellent knowledge of drug information research tools, clinical research methodologies, and outcomes evaluation.
