Job Summary

We are seeking a skilled and compassionate Nurse Care Specialist to join our team at ASSISI HOSPICE. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care and ensuring the well-being of our patients.

Key Responsibilities
  • Deliver Patient-Centric Care: Provide safe, competent, and individualized nursing care to patients, upholding nursing standards in documentation, treatment, patient monitoring, and infection control.
  • Coordinate Patient Care: Allocate equipment as necessary, communicate with patients and their families, and coordinate care with physicians and other healthcare professionals.
  • Facilitate Multidisciplinary Team Meetings: Participate in team meetings to ensure smooth delivery of patient care and services.
  • Ensure Patient Safety: Observe organization and legal requirements regarding storage and control of drugs and medical supplies, and report incidents to the Nurse Manager/Nurse Clinician.
  • Document Patient Information: Maintain accurate and up-to-date records of patient condition, treatment plans, and referrals.
  • Collaborate with Healthcare Team: Work collaboratively with other healthcare professionals to ensure comprehensive patient care.
  • Develop and Implement Care Plans: Monitor, reassess, and evaluate patient condition, and develop and implement care plans to meet patient needs.
  • Perform Nursing Care: Administer medications, perform wound dressings, and provide other nursing care as required.
  • Follow Up and Continuity of Care: Ensure continuity of patient care and treatment plans, and follow up with patients and families as needed.
Requirements
  • Education and Experience: At least a Diploma in Nursing from a local polytechnic and registered with the Singapore Nursing Board. Post-registration experience in a hospital or hospice setting is preferred.
  • Skills and Knowledge: Knowledge of IT and systems, passion for serving and providing excellent customer service, meticulous and accurate, good interpersonal and communication skills, and ability to perform under pressure.
