Case Manager Job Summary

To support the Care and Counselling Department in their areas of responsibility, including case management, administrative tasks, and support groups.

Responsibilities:

Clinical and Case Management:

  • Assist with case management after cases are screened by social workers.
  • Follow up on care arrangements and post-discharge referrals.
  • Explain long-term care schemes to patients and their families.
  • Conduct home visits as instructed by social workers.

Manage cases referred by AMKH Day Rehab Centre for ATF applications and serve as the point of contact for MSW services referrals.

Facilitate means testing for inpatients and patients attending dialysis at AMK-THKH Dialysis Centre.

Accompany patients for admission assessments for institution placement or centre-based services, and to other organizations for discharge planning.

Professional Development:

  • Engage in continuous learning through regular case discussions, supervision, and trainings (internal and external).

Programme Development:

  • Assist in preparing support groups, including admin and logistical support.

Administrative:

  • Ensure proper documentation of case files, referrals, and other related documents for case management and audit purposes.
  • Collect and submit regular statistics.
  • Man patient records and manage filing systems effectively.
  • Support the duties of the Admin Assistant when needed.

Requirements:

  • Diploma in Social Work / WSQ Diploma in Social Services.
  • Minimum 4 years of social service experience.
  • Proficient in MS Office.
  • Good communication and written skills.
