Job Description:

We are seeking a skilled and dedicated professional to join our team as a Contract Care Coordinator. This is a dynamic role that requires strong communication skills, attention to detail, and the ability to work effectively in a fast-paced environment.

As a Contract Care Coordinator, you will be responsible for managing inquiries on health services, coordinating requested services, and assessing customer needs using screening tools. You will also collaborate with internal stakeholders and external partners to ensure smooth service delivery, participate in regular program development and evaluation, and work closely with the Operations and Nurse Manager on client discharge from services.

  • Manage inquiries on health services and coordinate requested services

  • Collaborate with internal stakeholders and external partners on service referrals and follow-up actions
  • Conduct regular home visits and telephone follow-ups to engage with clients
  • Counsel and educate caregivers on providing effective care and support
Requirements:

To be considered for this role, you must possess a minimum higher nitec/diploma in any field and have relevant experience in the healthcare industry. Additionally, you should have strong interpersonal skills, be proficient in Microsoft Office, and have knowledge of quality improvement principles.

You will also need to have a good understanding of health promotion principles, be able to provide treatment planning and patient safety guidance, and have expertise in referrals and program development.
