Key Responsibilities
  • Provide pharmaceutical care to patients, including medication therapy management and drug information services.
  • Work collaboratively with healthcare teams to promote wellness, maintain current health, and intervene in acute or chronic illness.
  • Identify, resolve, and prevent drug-related problems.
  • Recommend and monitor patient drug therapy.
  • Interact with patients, family, and other healthcare providers to ensure continuity of services and achieve desired patient outcomes.
  • Ensure accurate and timely distribution of pharmaceuticals.
  • Provide education to patients, pharmacy staff, and other healthcare practitioners.
Requirements
  • Graduate from a School of Pharmacy recognised by the Singapore Pharmacy Council.
  • Full registration with the Singapore Pharmacy Council.
  • Valid membership with local professional body/association is preferred where applicable.
  • Experience as a Pharmacist in relevant clinical settings and demonstrated competency will be an advantage.
  • Candidates with 4 years of experience can qualify for the Senior Pharmacist position.
