Job Title: Senior Program Manager, Renal Care

This role oversees all operational aspects of Project Renalssance, a program aimed at demonstrating the effectiveness and scalability of early palliative care for advanced CKD patients. The ideal candidate will have experience in project management, staff performance management, and education.

Key Responsibilities:

  • Develop and implement clinical protocols for the management of advanced CKD patients.
  • Mentor Case Management officers on project management and staff performance management.
  • Monitor and evaluate staff performance, identifying areas for improvement.
  • Ensure educational opportunities are available for staff to develop professionally and contribute to the growth and development of the division.
  • Work with the Clinical Manager to coordinate appointments to the Kidney Supportive Care Clinic and referrals to community partners.
  • Apply understanding of prevention of medico-legal issues related to patient care.
  • Participate in quality improvement activities.

Requirements:

  • Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology, or General Science.
  • Certificate/Diploma in Renal Nursing advantageous.
  • Registered Nurse with at least 3 years of clinical experience.
  • At least 5 years prior working experience in a healthcare environment, assuming managerial duties before.
  • Knowledge of clinical skills and critical thinking in delivery of patient care advantageous.
  • Ability to resolve problems in all aspects of implementation of Project Renalssance, including operational, IT, and clinical-related issues.
  • Leadership skills to motivate staff, ensure knowledge retention, and succession planning.
  • Good interpersonal and communication skills.
  • Computer literacy skills – MS Office (Word/Excel/PowerPoint).
  • Effectively bilingual.
