Our organization is committed to delivering high-quality nursing services to the elderly in a home-based setting. We are seeking an experienced and compassionate Staff Nurse to join our interdisciplinary team, providing exceptional patient care and support.


The ideal candidate will have a strong background in gerontological nursing, with extensive clinical knowledge and skills. They will be responsible for conducting regular home visits and comprehensive assessments of frail elders, as well as developing and implementing effective care plans.


Key Responsibilities:
  • Perform comprehensive assessments of clients' medical and nursing needs.
  • Develop and implement individualized care plans, working closely with the interdisciplinary team.
  • Provide high-quality nursing care and interventions to promote optimal health outcomes.
  • Collaborate with healthcare professionals to ensure seamless care transitions and communication.
  • Maintain accurate and up-to-date records of client progress and outcomes.

Requirements:
  • Professional Certificate/Specialist Diploma/Advanced Diploma in Nursing, preferably gerontological nursing.
  • State registration with the Singapore Nursing Board (SNB) and valid Practising Certificate.
  • 2-3 years experience in social services or eldercare sector.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse stakeholders.
  • Patient-focused approach, able to prioritize client needs and deliver high-quality care.
