The ideal candidate will be responsible for conducting patient visits, monitoring conditions, conducting tests, and writing reports for physicians. They should be able to address caregivers' concerns and make adjustments to patients' in-home management plans.

Responsibilities
  • Conduct screening referrals and assessments of potential clients.
  • Conduct home environment assessments.
  • Develop individualised care plans involving caregivers and family members for clients.
  • Review clients' ICPs and conditions periodically and revise the ICP when necessary.
  • Administer medication and insulin, complete blood pressure, glucose, urine, and stool tests.
  • Collaborate with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients.
  • Conduct health education and training for clients, caregivers, and the community.
  • Test for muscle weakness, bedsores, and signs of infections.
  • Provide basic care management services.
  • Link and match clients to required services.
  • Advise on government subsidies application.
  • Collect payments and issue receipts for delivered services.
  • Assist doctors during home visits as needed.
  • Participate in multidisciplinary discussions.
  • Develop discharge plans for clients no longer requiring service.
  • Discuss caregiver needs in delivering care to loved ones and self-care.
  • Assist caregivers in navigating eldercare and caregiver landscape.
  • Link and match caregivers to appropriate services.
  • Assist with administrative work (e.g., data entry, invoice generation, minutes taking).
  • Support new and less experienced staff.
  • Participate in outreach and roadshows to promote the programme in the community.
  • Develop close working relationships with internal and external stakeholders.
Requirements
  • Diploma, Advanced Diploma, Higher Diploma, or Graduate Diploma in Nursing or a similar field.
  • At least 3 years of working experience in the related field is required.
  • Valid Singapore Nursing Board License (Staff Nurse).
  • Patience and compassion in serving elderly residents.
  • Team player with strong communication, interpersonal, and organisational abilities.
  • Capable of working independently and under time constraints.
  • Proficient in Microsoft Office applications.
