Job Description:

We are seeking a highly skilled and compassionate Social Work Assistant to join our team. As a Social Work Assistant, you will play a vital role in supporting the Care & Counselling Department with various responsibilities.

Clinical and Case Management:

You will assist with case management after screening by MSWs, follow up with care arrangements and post-discharge referrals, and explain long-term care schemes to patients and NOKs. You will also conduct home visits as instructed by MSWs, manage cases referred by AMKH Day Rehab Centre for ATF applications, and serve as the contact for MSW referrals.

Furthermore, you will facilitate means testing for inpatients and dialysis patients at AMK-THKH Dialysis Centre, assist patients with admission assessments and discharge planning, and engage in continuous learning through case discussions, supervision, and training.

Professional Development:

You will have opportunities to develop your professional skills through continuous learning, including case discussions, supervision, and training.

Programme Development:

You will assist in preparing support groups, including administrative and logistical support, to provide a safe and supportive environment for patients.

Administrative:

You will ensure proper documentation of case files, referrals, and related documents, collect and submit regular statistics, manning the Care and Counselling Reception on Saturdays, and perform other administrative duties as assigned.

Requirements:
  • Diploma in Social Work or WSQ Diploma in Social Services
  • Fresh graduates are welcome
  • Minimum 4 years of social service experience
  • Proficient in MS Office
  • Good communication and written skills
