**Job Description:**

CADMUS RESOURCES is seeking a skilled Palliative Care Specialist to join our team. In this role, you will provide expert care and support to patients and their families, ensuring they receive the best possible outcomes.

About the Role:

  • Provide socio-economic evaluations to help patients address practical concerns.
  • Offer individual and family counselling to navigate personal and ethical dilemmas.
  • Work with community organizations to achieve optimal end-of-life goals.
  • Engage families in the care process and prepare them for the end-of-life stage.
  • Provide bereavement information, education, and support to help families cope with loss.
  • Attend Interdisciplinary Patient Care Conferences to discuss patient plans.
  • Counsel patients and families on financial and discharge care plans.
  • Perform field work to alleviate or resolve medico-social problems.
  • Prepare social and related reports for management and institutions.
  • Present cases for discussion and engage actively on formal platforms.
  • Maintain constant networking with MSW, Counsellor, staff nurses, and medical staff.

About You:

  • You hold a Degree in Social Work and/or Master in Counselling preferably.
  • You are registered with the Singapore Association of Social Workers for Social Workers or Singapore Association for Counselling (SAC).
  • You have at least 4 to 9 years of relevant experience in a healthcare environment preferably.
  • You are bilingual and able to communicate in local dialects preferred.
