Job Title: Healthcare Programme Coordinator

The role of the Healthcare Programme Coordinator involves providing strategic support to facilitate the implementation of programmes across various departments. This includes assisting in programme planning, coordination and evaluation activities.

Main Responsibilities:
  • Collaborate with programme teams to develop and implement programme frameworks.
  • Screen referrals and direct them to suitable service tiers based on patient needs.
  • Engage eligible patients and ensure their successful enrolment into the programme.
  • Track patient progress and update referring clinicians accordingly.
  • Maintain relationships with external agencies and partners.
  • Implement seamless referral workflows and operations.
  • Contribute to enhancing programme services and quality.
  • Support project team discussions and stakeholders' engagements.
  • Act as the point-of-contact for programme-related queries.
  • Monitor programme indicators and provide progress updates to stakeholders.
Requirements:
  • Degree in Nursing, Allied Health or related discipline.
  • Independent with good initiative.
  • Excellent interpersonal and communication skills.
  • Meticulous and organized.
  • Proficient in Microsoft Office applications.
  • Relevant working experience in healthcare sector is advantageous.
