Job Title: Compassionate Healthcare Advocate

The role of a medical social worker plays a vital part in supporting our residents through person-centred care and ability centred care.

Key Responsibilities:

  • Casework Management
    • Provide comprehensive case management, including care assessments and intervention
  • Financial Assistance
    • Facilitate financial assistance for new residents in line with MediFund guidelines
    • Conduct financial assessments and reviews to ensure continuous funding support for residents
  • Admissions and Occupancy
    • Work with caregivers and referring parties to perform intake assessments
    • Coordinate the end-to-end admission process
  • Other Key Functions
    • Networking with healthcare facilities, social agencies and community partners
    • Active member of relevant committees, including Pandemic Response Team
    • Provide caregiver support and strengthen engagement with next-of-kin
    • Carry out ACP/PPC documentation

Requirements:

  • Degree in Social Work or equivalent qualification
  • Registered or eligible for registration with the Singapore Association of Social Workers
  • Minimum 2 years of relevant experience preferred
  • Familiarity with Medifund, MFEC, PGDAS, IDAPE, Eldershield/Careshield, and Medisave
  • Strong administrative, writing, and conceptualisation skills
  • Compassionate, mature, and team-oriented with excellent interpersonal skills
  • Knowledge of medical social work
  • Talents in wellness or creative facilitation are a plus
