Job Overview:

The Chronic Disease Management Operations Lead is a pivotal role, responsible for overseeing the day-to-day operations of a transformative programme.

This position supports the Director in driving programme execution, managing timelines, coordinating across workstreams and partners, and ensuring smooth delivery to end users.

The Programme Lead plays a key role in operational planning, service design, and performance monitoring.

Key Responsibilities:

  • Collaborate with design consultants to support the design, development, and implementation of enhanced chronic disease management programmes.
  • Lead daily operations of the programme, including planning, tracking timelines, and managing task pipelines.
  • Assist in budgeting and financial reporting for the programme.
  • Coordinate stakeholder and partner engagement activities.
  • Develop and maintain a programme dashboard to monitor key success metrics and operational KPIs.
  • Translate strategic outputs from the development team into actionable workflows for the service operations team.
  • Evaluate efficient coordination and communication among service team members to support operational excellence.
  • Contribute to documentation of programme tools, knowledge base, operational manuals, and staff training guides.
  • Support the Director in the design and implementation of the broader healthcare ecosystem.

Qualifications:

  • Bachelor's degree or equivalent in a relevant field.
  • At least 8 years of programme management experience in healthcare, service delivery, or related industries.
  • Proven experience engaging with medical stakeholders and/or working within the healthcare sector is highly desirable.
  • Minimum of 3 years in a leadership or managerial capacity.
