Telecare Nurse

We are seeking a highly skilled and compassionate Telecare Nurse to join our team at Khoo Teck Puat Hospital (KTPH). As a Telecare Nurse, you will play a critical role in coordinating patient care and managing care plans to ensure the best possible outcomes for our patients.

• Coordinate Care and Manage Care Plans
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes.
• Perform Assessments and Link with Community Services
  • Conduct comprehensive assessments of patients' functional and psycho-social needs.
  • Link patients with community resources and services to support their care and well-being.
• Provide Education and Support
  • Deliver patient and family education on chronic disease management and healthy lifestyle.
  • Provide emotional support and guidance to patients and families navigating the healthcare system.
• Anticipate Care and Manage Escalations
  • Identify early signs of exacerbations of chronic diseases and develop escalation plans.
  • Provide 24/7 tele-triage services to receive incoming calls from patients and assess the urgency of conditions.

• Education and Experience
  • Diploma, Advanced Diploma, Specialist Diploma, or Bachelor Degree in Nursing.
  • Minimum 2 years of experience in nursing, preferably in a telehealth or care coordination role.
• Skills and Competencies
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.

This is a full-time position with a 3-rotating shift schedule. The successful candidate will be required to work at our Yishun location.
