Job Title

We are seeking a motivated and organized professional to join our case management team.

Key Responsibilities:
  • Manage nursing home referrals and admissions, ensuring smooth resident transitions
  • Assist residents and families with financial assistance applications, ensure timely Household Means Testing updates, and generate monthly reports
  • Support resident's journey from admission till end of life, address resident and family enquiries related to admissions, financial schemes, and general care
  • Maintain accurate resident records and profiles in digital systems, track outstanding payments, manage resident financial information, and liaise with banks or HDB when needed
Requirements:
  • A diploma in a relevant field (healthcare, social services, or administration)
  • At least 1 year of experience in healthcare, eldercare, or community service settings in Singapore
  • Familiarity with geriatric care, nursing home operations, or community services
  • Proficient in G Suite applications (Docs, Sheets, Slides, Gmail)

About This Role:
