Pharmacist Job Description

The role of a pharmacist is to ensure the safe and effective use of medications. This includes reviewing patient medication charts, contacting prescribing doctors when necessary, and dispensing medications to patients.

Key responsibilities include:

  • ">
  • Reviewing each patient's medication chart or prescription for appropriateness and contacting the prescribing doctor when an intervention is needed;">
  • Ensuring that all orders are clear and specific, doses are prescribed within therapeutic limits, any potential adverse reactions and interactions are identified, and fulfilling all drug orders accurately and in a timely manner;">
  • Dispensing medications to patients, explaining the indications for drug therapy and instructions for administration;">
  • Providing drug information and lectures to other healthcare professionals;">
  • Delegating, overseeing, and reviewing the work of assigned subordinates;">
  • Ensuring that all sales and supply of medicinal products comply with legal requirements;">
  • Maintaining records pertaining to all phases of drug receipt, supply, storage, and distribution as required by the hospital and/or legal authorities;">
  • Representing the department in various clinical committees and performing other job-related duties as assigned;">
  • Reviewing pharmacy procedures and policies periodically and implementing schemes to improve pharmacy workflow;">
  • Supervising performance, conducting appraisals, writing reports, and making recommendations for confirmation, promotion, and training needs of subordinates;">
  • Actively participating in Pharmacy education and development programs;">
  • Practicing continued reading of journals, publications, etc., for professional advancement;">
  • Being responsible for the implementation and adherence to policies, practices, and regulations established by the department, hospital, and external regulatory agencies pertaining to pharmacy;">
  • Providing relevant training to pre-registration pharmacists and pharmacy students;">
  • Supporting the hospital by participating actively in the various activities organized by the hospital;">
  • Conducting clinical review for appropriateness of medication orders;">
  • Ensuring operational efficiency to meet desired turnaround time;">
  • Supervising performance and conducting appraisal;">
  • Involving in national or institutional workgroups and projects;">
  • Meeting the educational and training requirements specified below.">

Education and Training Requirements

Applicants must have a Bachelor of Science degree in Pharmacy from an accredited University recognized by the Singapore Pharmacy Council and be registered with the Singapore Pharmacy Council.
