We are seeking a dedicated and compassionate individual to lead patients and families navigating complex healthcare journeys.

Key Responsibilities:
  • Provide individualized assistance to overcome healthcare system barriers and improve continuity of care.
  • Identify high-risk patients and ensure access to resources; follow up with patients to reduce unnecessary ED visits and admissions.

Requirements:
  • Bachelor degree in Health Sciences; degree in Nursing or Allied Health relevant to the job preferred.
  • Candidates with relevant working experience in a similar setting preferred.
  • Excellent interpersonal and communication skills; ability to work independently and as part of a team.
