Job Description

The Freelance Pharmacist plays a pivotal role in delivering timely, safe and effective medication therapy to patients.

  • Reviews medication orders for appropriateness and accuracy, including contraindications, allergies, dose, route, frequency, duplicate therapy, and formulary status.
  • Interprets and clarifies physician medication and IV orders, ensuring seamless integration with the medication reconciliation process for inpatients and outpatients.
  • Verifies patient medication orders with physicians to ensure dose, route, name, and frequency of administration are accurate.
  • Prepares sterile and non-sterile preparations in compliance with established compounding regulations.
  • Monitors patients on specified target drugs and initiates pharmacokinetic dosing protocols to optimize treatment outcomes.
  • Evaluates drug therapy plans for effectiveness and makes informed recommendations to clinicians.
  • Dispenses medications, provides instructions on proper use, and conducts comprehensive patient counseling sessions.
  • Identifies, monitors, and reports adverse drug events and medication errors in accordance with established procedures and regulatory requirements.

Required Skills and Qualifications

Key qualifications include:

  • Proficiency in reviewing medication orders and interpreting physician medication and IV orders.
  • Ability to prepare sterile and non-sterile preparations in compliance with established compounding regulations.
  • Experience in monitoring patients on specified target drugs and initiating pharmacokinetic dosing protocols.
  • Strong communication and interpersonal skills for working effectively with healthcare teams.
