Job Title: Community Transformation Specialist

The role of a Community Transformation Specialist plays a pivotal part in driving positive change and enhancing the lives of individuals, families, and communities facing transitional challenges.

  • This is achieved through a community-centric approach and the delivery of targeted interventions, including social work services, home ownership support, livelihood development, and community mobilisation.
  • The ultimate aim is to empower communities to break the cycle of poverty and achieve sustainable progress.

Responsibilities:

Implementing Code of Social Work Practice Standards
  • BPSS and FAST implementation for assigned supervisees within the context of the Social Service Network (SSNet).
  • Maintenance of accurate and confidential family records, documenting progress, interventions, and outcomes on required platforms such as SSNet, CaseConnect, and Family Transformation Management System (FTMS).
Providing Quality Community-Focused Casework and Counselling Services
  • Managing cases from intake, assessment, documentation, referrals, and closure.
  • Participation in networking and case management meetings with key stakeholders.
  • Conducting relevant assessments, including FAST, BPSS, and other financial or risk evaluations.
Enabling Families to Achieve Poverty Transformation Through Specialist Interventions
  • Offering guidance on specialist interventions to families, encompassing comprehensive assessments, action plan development, goal-setting, and progression towards achieving home ownership and/or good livelihood and/or debt management and clearance.
  • Implementation of Home Ownership (KeyStart) and Good Livelihood Programmes, including enrolment into respective programmes where relevant and conducting necessary activities.
Delivering Effective Programmes Including Groupwork and Community Outreach
  • Collaboration with Senior Social Worker, Lead Social Worker, and/or Executive Director to devise a holistic suite of programmes and services customised to meet the needs of clients in their immediate area of influence/community.
