The Service Delivery Specialist will collaborate with other divisions and external partners on service referrals, submit referral forms, and ensure follow-up on action points to guarantee timely resolutions.

In addition, the role involves conducting regular home visits and telephone follow-ups to engage with clients, as well as collaborating with social and health partners to recommend appropriate care programs.

Caregivers will be counselled and educated on providing effective care, support, and available financial assistance through strategic partnerships.

The Service Delivery Specialist will work closely with the Operations and Nurse Manager on client discharge from services and participate in regular program development and evaluation to improve service offerings and enhance the overall customer experience.
