Job Overview:

This role involves facilitating dementia therapy, providing home care services, and delivering professional, patient-centered care. The successful candidate will be responsible for ensuring the comfort, safety, and adherence to therapeutic goals of our clients.

Caregiver Support:

  • Foster a collaborative environment among caregivers and healthcare professionals to enhance patient outcomes
  • Develop and maintain strong relationships with patients, families, and caregivers to promote trust and open communication
  • Ensure all services comply with healthcare regulations, industry standards, and company policies
  • Stay up-to-date with industry developments, best practices, and research findings to inform practice and improve patient outcomes

Key Responsibilities:

  • Provide high-quality care that promotes independence and enhances quality of life for our clients
  • Facilitate care planning and goal setting in collaboration with patients, families, and multidisciplinary teams
  • Implement evidence-based practices and interventions to improve patient outcomes and quality of life
  • Document patient progress, therapy sessions, and care plans accurately and timely following company policies and regulatory standards
  • Collaborate with multidisciplinary teams to coordinate care and services, ensuring seamless transitions and continuity of care

Requirements:

  • GCE 'O' Level, Nitec, Diploma or equivalent
  • Experience in healthcare at home and assisted living environments is preferred but not required
  • Excellent observational and problem-solving skills
  • Compassionate, patient, and dedicated to providing high-quality care
  • Ability to work independently and manage time effectively
  • Excellent communication, strong interpersonal skills, a positive attitude towards work, and being a team player
  • Ability to write and communicate in English
  • Computer literacy

Work Arrangements:

  • Flexible working arrangements with work-life balance
  • No shift work
  • 5.5 days; Working hours: Mon to Fri 9am - 6pm, Sat 9am - 1pm
