The successful candidate will be responsible for providing high-quality social services to our clients, including case management, community health promotion, and administrative support.

Key Responsibilities
  • Case Management: Assess the community health and social care needs, coordinate support and service placement, develop and pilot work flows, protocols and processes that facilitate the integration of social and health care for residents, and develop and implement calendar plans of social and health activities for seniors.
  • Community Health Promotion: Employ strengths-based practice models to work with service users, identify their physical, emotional, and social needs, conduct care assessments, formulate individualized care plans, and help service users reach their goals and action plans.
  • Administrative Support: Assist in the supervision and/or training of persons/students on placement, conduct presentations as and when required to external parties, keep up-to-date statistics and records of the services rendered, and perform general administrative duties.
Requirements
  • Graduate Diploma in Social Work or Degree in Social Work
  • Relevant experience in Social Service Sector or related field is an advantage
  • Training provided
