About the Role:

We are looking for a Care Consultant to join our team at Agency for Integrated Care Pte. Ltd., a leading provider of community health services in Singapore.

Job Description:
The successful candidate will be responsible for:

  • Carry out comprehensive assessments of clients' care needs, ensuring timely referrals and placement in suitable care settings.
  • Manage day-to-day operations, delivering prompt and quality customer service.
  • Engage with clients and caregivers to identify care needs and develop personalized plans, linking them to relevant services and support.
  • Assess and match clients with financial schemes, facilitating applications to support their care expenses.
  • Solicit feedback from clients and caregivers to review service standards and improve customer satisfaction.
  • Liaise with hospitals, referring entities, and service providers to facilitate client placements in desired care facilities.
  • Participate in case discussions and client/family conferences with relevant stakeholders.
  • Contribute to initiatives, projects, and events to enhance service delivery.
  • Collaborate as a subject matter expert to continuously improve systems, processes, and services.
  • Ensure accurate documentation of all transactions into the IT system or case notes.


Requirements:
To be successful in this role, you will need:

  • A professional qualification in Nursing or Allied Health disciplines relevant to the job.
  • At least 3 years of experience in related fields in acute and/or community healthcare settings in Singapore.
  • Knowledge of geriatrics, community services, and financial schemes is an advantage.
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint).
  • Good problem-solving skills and attention to detail.
  • Pro-active work attitude with excellent interpersonal, communication, and organizational skills.
  • Able to work effectively and harmoniously with internal and external stakeholders.
  • Keen to stay updated on clinical assessment tools, community services, and schemes.
  • Able to work independently, multitask effectively under pressure, and adapt to changes.
  • Able to comply with process/manuals, advisories, guidelines, policies, legislations/Acts relevant to healthcare and the area of work.


Estimated Salary: SGD $5,500 - $7,000 per month, depending on qualifications and experience.
