Job Overview

We are seeking a skilled professional to lead our case management team and deliver high-quality services in case management, programs, community outreach, respite carer recruitment, and publicity.

Main Responsibilities:

  • To effectively supervise case workers and interns to achieve good results in case management, programs, community outreach, respite carer recruitment, and publicity, including assisting the Director (Clinical Services) in the operations of Sanctuary Care Management.
  • Spearheading case movement through regular team meetings, case review sessions, etc.
  • Identifying at-risk situations, developing strategies, and intervention plans to address child safety issues and wellbeing.
  • Monitoring and evaluating the effectiveness of case management and other support programs and activities extended to respite carer families.
  • Developing relevant Standard Operating Procedures (SOPs) for the department.
  • Conducting in-depth assessments and producing written reports on the suitability of applicants for the role as respite carers, presenting findings to management, and following up on decisions and recommendations.
  • Matching respite families with foster children based on factors such as CANs assessments and/or age, race, religion, as well as the foster parent's capability in meeting the child's needs.
  • Conducting all necessary checks, including announced and unannounced home visits, to ensure that the respite carer can safeguard and promote the child's safety, stability, and wellbeing.
  • Organizing and participating in inter-agency efforts to support the welfare and development of foster children/CYP.
  • Coordinating a 24-hour hotline for respite carer duty schedules and ensuring that the team follows up on outcomes.
  • Supervising and supporting foster placements to prevent breakdowns in foster children's existing care arrangements.
  • Producing accurate recordings on cases and submitting reports punctually to Boys' Town and other partners.
  • Participating in road shows and other community outreach events as needed.
  • Meeting regularly with the Head Sanctuary Care/supervisor to assess case progress and program outcome/effectiveness.
  • Managing and supporting volunteers interested in providing their service to Sanctuary Care.
  • Contributing relevant articles and social media content to promote mental health.
  • Coordinating marketing and publicity projects to promote Sanctuary Care services.
  • To support and conduct research related to practice.
