Role Summary:
Community Care SpecialistWe are seeking a dedicated and compassionate Community Care Specialist to join our team. As a key member of our healthcare services, you will be responsible for delivering high-quality care to clients in their homes and communities.
Key Responsibilities:
- Nursing Care Delivery
- Conduct comprehensive assessments of clients' health needs
- Develop and implement personalized care plans to meet clients' unique needs
- Provide ongoing nursing care and support to clients
- Care Coordination
- Collaborate with multidisciplinary teams to ensure seamless care delivery
- Coordinate with healthcare providers to ensure timely access to medical services
- Monitor client progress and make adjustments to care plans as needed
- Health Education and Promotion
- Provide education and guidance to clients and their families on healthy living practices
- Encourage clients to take an active role in their health and well-being
- Promote community resources and services that support clients' needs
Requirements:
- Qualifications
- Degree or Diploma in Nursing
- Registered Nurse with Singapore Nursing Board
- Experience
- Minimum 3 years of relevant work experience preferred
- Skills
- Strong communication and interpersonal skills
- Able to work independently and as part of a team
