Compassionate Palliative Care Specialist Wanted

We are seeking a qualified and experienced palliative care staff nurse to join our team. The ideal candidate will have a strong background in nursing and be able to work independently in a fast-paced environment.
Job Responsibilities:- Perform home visits to patients with chronic illnesses or palliative care needs.
- Conduct thorough assessments of patients' physical and emotional needs.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Provide education and support to patients and their families on various aspects of palliative care.
- Current SNB and BCLS certification.
- Minimum 2 years of experience in palliative care nursing.
- Strong communication and interpersonal skills.
- Able to work effectively in a team environment.
