Job Title: Clinical Instructor

About the Role

We are seeking a highly skilled and experienced Clinical Instructor to join our team. As a Clinical Instructor, you will be responsible for delivering high-quality patient care in collaboration with multi-disciplinary teams (MDTs). You will implement evidence-based practices to achieve desired care outcomes.

You will have a key role in facilitating admissions, transfers, and discharges, ensuring timely discharge planning according to assessed needs and health conditions. You will also follow up and evaluate patients' progress according to planned care and readiness for discharge.

Key Responsibilities:
  • Facilitate admissions, transfers, and discharges
  • Ensure timely discharge planning
  • Follow up and evaluate patients' progress
  • Collaborate with MDTs to deliver evidence-based care
  • Monitor patients for changes in condition and escalate as required

As a Clinical Instructor, you will be part of a dynamic team that is committed to delivering exceptional patient care. Requirements

To be successful in this role, you will need to have:

  • A minimum degree in Nursing
  • At least 5 years of relevant inpatient experience
  • Senior nursing experience, preferably in gerontology/palliative care
  • An Advanced/Specialist Diploma in Geriatric/Community Nursing & Long-Term Care settings
