Job Title: Nursing Specialist - Home Care

We are seeking a compassionate and skilled Home Care Nurse to provide high-quality nursing care to clients in their own homes. As a key member of our team, you will work collaboratively with internal and external stakeholders to deliver efficient and effective care.

The successful candidate will possess excellent communication and interpersonal skills, with the ability to work independently and as part of a team. A strong understanding of nursing practices and regulations is essential, along with the ability to adapt to changing situations.

Key Responsibilities:
  • Conduct home visits to assess clients' needs and develop personalized care plans.
  • Perform various nursing procedures in the home environment, prioritizing safety and implementing necessary precautions.
  • Provide patient education to empower clients to manage their health effectively.
  • Liaise with healthcare partners to ensure seamless care coordination.
  • Train caregivers to support clients' well-being.
  • Carry out discharge planning to ensure smooth transitions for clients.
Additional Duties:
  • Screen medical and nursing referrals from external agencies.
  • Engage in new initiatives and projects as assigned.
  • Share knowledge on caring for clients with complex medical or social issues.
