Care Advisor

We are seeking a dedicated and compassionate Resident Care Specialist to support our residents.
The successful candidate will have a strong background in social work, with experience in developing and implementing care plans that cater to the unique needs and circumstances of each resident. The ideal candidate will be able to develop individualized care plans for approximately 50 residents based on their unique requirements and preferences. This role requires close monitoring of progress, making adjustments as necessary.
Key Responsibilities:
- Develop Individualized Care Plans: Create personalized care plans for residents based on their specific needs and circumstances.
- Execute and Monitor Progress: Implement and monitor the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
- Design and Implement Programs: Develop programs and activities that meet the residents' care needs, promoting their physical, emotional, and psychological well-being.
- Prepare and Submit Reports: Prepare and submit regular review reports and interview records for residents.
Requirements:
- Degree in Social Work: A degree in Social Work from an accredited institution is required.
- Experience in Social Service Sector: Preferably experience in the Social Service Sector or related field.
