Key Responsibilities
  • Conduct comprehensive assessments of patients' psycho-social needs and develop personalized care plans
  • Provide counseling and support to patients and their families, addressing concerns related to mental health, relationships, and daily living skills
  • Collaborate with interdisciplinary teams to ensure seamless care delivery and optimize patient outcomes
  • Identify and mobilize community resources to support patients' reintegration into society
Requirements
  • Bachelor's degree in Social Work or related field; Master's degree preferred
  • Minimum 1 year of experience in clinical social work, preferably in an acute care setting
  • Strong assessment and intervention skills; ability to work effectively with diverse populations
  • Excellent communication and interpersonal skills; ability to build rapport with patients, families, and healthcare professionals
Working Arrangements

Multiple work locations available, including Yishun (North); non-shift working hours
