We are seeking an exceptional individual to fill the position of Advanced Care Practitioner. As a vital member of our team, you will be responsible for delivering high-quality nursing care and management services to our clients.
About the Role- Develop and implement comprehensive care plans tailored to each client's unique needs.
- Collaborate with healthcare professionals to ensure seamless care coordination and effective patient outcomes.
- Provide education and support to clients and their families to empower them in managing their health conditions.
- Maintain accurate records and reports to track client progress and meet organizational requirements.
- Conduct thorough assessments to identify client needs and develop personalized care plans.
- Administer medication and perform medical procedures as required.
- Monitor client health status and make referrals to specialist services when necessary.
- Establish strong relationships with clients and their families to promote trust and open communication.
- Develop and implement self-management plans to empower clients in managing their health conditions.
- Provide ongoing education and support to clients and their caregivers.
- Assist in planning and facilitating Homecare services to meet client needs.
- Collaborate with colleagues to achieve targets and manage client outcomes.
- Maintain accurate records and reports to track client progress.
- Registered Nurse with a Diploma in Nursing.
- Minimum 3 years of relevant nursing experience.
- Competent to independently provide nursing care according to established protocols.
- High integrity, resilient, and passionate about nursing and helping others.
- Effective communication and interpersonal skills.
