About the Role:

The TSAO Foundation is seeking a highly skilled and compassionate Staff Nurse to join our team. As a Staff Nurse, you will play a vital role in providing high-quality home-based nursing services to our frail and home-bound elderly clients.

Key Responsibilities:

  • Conduct regular home visits and perform comprehensive nursing assessments to identify the needs of our clients.
  • Triage client conditions and respond promptly to address any issues that may arise.
  • Collaborate with our interdisciplinary team to develop and implement client care plans that meet their unique needs.
  • Communicate effectively with clients and caregivers to ensure active participation in their care.
  • Evaluate the effectiveness of interventions and revise care plans as needed.
  • Manage and monitor client medical and nursing needs, ensuring timely interventions and referrals when necessary.
  • Maintain accurate documentation of client progress and outcomes.
  • Assist our doctors in charge and perform nursing procedures as required.
  • Provide education and training to clients, caregivers, and team members to enhance their knowledge and skills.
  • Perform administrative duties in accordance with internal and external agency guidelines and SOPs.

Requirements:

  • Professional Certificate/Specialist Diploma/Advanced Diploma in Nursing, preferably with training in gerontological nursing.
  • State registration with the Singapore Nursing Board (SNB) and a valid Practising Certificate in Singapore.
  • Preferably 2-3 years of working experience in the social services or eldercare sector.
  • Extensive clinical knowledge and skills.
  • A passion for working with the elderly and caregivers.
  • Flexibility, energy, and the ability to multitask.
  • Excellent communication and interpersonal skills.
  • Bilingual in 2 or more local dialects, as you will be required to communicate with elderly individuals within the community.
