
Pharmacist

A clinical pharmacist is required to work in an office hour setting. The role involves reviewing medication orders for appropriateness, processing drug orders, dispensing medication to patients and supplying drug stocks to nursing units.
Key Responsibilities:
1. Review medication orders for appropriateness
2. Process drug orders
3. Dispense medication to patients
4. Supply drug stocks to nursing units
5. Counsel patients on their medication
6. Provide drug information to doctors, nurses, and patients
7. Perform extension hours, night duties, and on-call duties as assigned by duty roster (if applicable)
8. Assist in the preparation of ad-hoc items required on prescription orders
9. Adhere to legal and hospital policy requirements
10. Participate in continuing professional education (CPE)
11. Assist in the training and supervision of pharmacy assistants and technicians
12. Participate in quality assurance activities (i.e. ward inspection/expiry checks) as assigned
13. Perform medication-therapy related duties (i.e. supply and distribution of controlled drugs, adverse drug reactions reporting, drug usage monitoring, etc.)
14. Participate in medication reconciliation rounds and specialized counselling (warfarin and inhalation techniques counselling)
15. Assist in providing clinical experience to pre-registration pharmacists
16. Maintain records relating to drug supply, storage, and distribution as required by hospital and legal regulations (i.e. JCI, MOH standards)
17. Perform the function of purchasing pharmacist on a rotational basis (if assigned/if applicable)
18. Assist in reviewing and updating procedure/protocols/intranet materials on a regular basis
19. Participate in approved pilot schemes/new services (if applicable)
20. Represent pharmacy in various hospital projects as assigned by in-charges (if applicable)
Requirements:
- A Degree in Pharmacy and registered with the Singapore Pharmacy Council
- Experience in dispensing in an outpatient setting is preferred
