Job Title: Clinical Instructor

We are seeking a skilled and dedicated Clinical Instructor to join our team. As a Clinical Instructor, you will be responsible for implementing patient care in collaboration with multidisciplinary teams and ensuring that patients receive high-quality care according to their assessed needs and health conditions.


Key Responsibilities:
  • Implement patient care in collaboration with the multidisciplinary teams (MDT) and according to assessed needs and health condition(s).
  • Facilitate admissions, transfers, and discharges; ensure timely discharge planning is done upon patient's admission and appropriate to condition.
  • Follow up and evaluate patients' progress according to planned care and readiness for discharge.
  • Collaborate with the MDT to deliver evidence-based care to patients to achieve desired care outcomes.
  • Monitor patients for changes in condition, recognize early signs of clinical deterioration using appropriate physiological assessments, and escalate as required to enable timely implementation of interventions.
  • Participate and contribute actively in MDT rounds, execute physicians' treatment, and investigation orders promptly.
  • Prepare and assist physicians with bedside patient procedures and treatments.
  • Communicate and discuss patients' progress and response to treatment in collaboration with the MDT.
  • Demonstrate clinical skills, knowledge, critical thinking, and reasoning in the delivery of patient care.
  • Assist nursing teams to problem-solve if there are any patient/clinical/or operational issues and seek appropriate assistance/escalate to Senior Nurse Manager (SNM)/Nurse Manager (NM).
  • Deliver patient and/or family education on disease progression, prevention, and rehabilitation and assess understanding of condition to enable participation in self-healthcare management.

Required Skills and Qualifications:
  • Min Degree in Nursing with min 5 yrs of relevant perm inpatient & senior nursing experience, preferably in gerontology/palliative care with Advanced/Specialist Diploma in Geriatric/Community Nursing & Long-Term Care settings.
