Job Summary

We are seeking a highly skilled and experienced Senior/Medical Social Worker to join our team at KK Women's and Children's Hospital Pte. Ltd. The successful candidate will be responsible for providing high-quality clinical and community social work services to families with young children.

Key Responsibilities
  • Case Management: Ensure high standards of child-centric clinical and community practice within a trans-disciplinary framework.
  • Team Supervision: Provide supervision, coaching, and social work input to a multi-disciplinary team.
  • Client Support: Provide direct or indirect socio-emotional support, assessment, and referral for financial assistance to mothers and children from early conception to the first few years of the child.
  • Case Documentation: Maintain overview of team's cases, triage cases for presentation, and ensure case documentations are promptly completed and filed.
  • Intake and Risk Assessment: Conduct intake interviews and risk assessments, and assign cases to respective keyworkers for service delivery.
  • Community Partnerships: Assess clients for needs and facilitate referrals to community partners or services for further follow-up.
  • Home Visits: Conduct home visits as required.
Requirements
  • Education: Degree in Social Work or Postgraduate Degree in Social Work.
  • Experience: At least 3-5 years of direct social work experience practice, especially in working with families with young children.
  • Skills: Fast and efficient worker with the ability to multitask, registered social worker with the Social Work Accreditation Board (SWAB), and supervisory experience is preferred.
