Patient Care Specialist

As a vital part of our care team, you will provide nursing care that matters to patients in need. You will work in a supportive environment where staff collaborate effectively, providing holistic care and education activities to assigned patients.
### Job Responsibilities
• Provide high-quality patient care, treatment, and patient education to ensure optimal patient outcomes.
• Work collaboratively with the healthcare team to achieve common goals.
• Ensure accurate documentation of patient information and progress.
• Participate in quality improvement initiatives to enhance patient care and services.
### Essential Qualifications
• Hold a minimum NiTEC / Certificate in Nursing or equivalent recognized by the Singapore Nursing Board.
• Possess valid Registered Nurse practising certificate issued by the Singapore Nursing Board.
• Hold valid BCLS (Basic Cardiac Life Support) certification.
• Be comfortable with shift work and open to deployment within two work sites under SingHealth Community Hospitals.

• Assessing patient needs and developing individualized care plans.
• Quality Improvement techniques to enhance patient care and services.
• Treatment and patient education activities to promote patient well-being.
• Healthcare concepts and practices, including infection control and prevention strategies.
