About the Role:
This is an exciting opportunity to join our team as a Patient Care Specialist, working closely with patients and their families to ensure seamless transitions from hospital to home.
Duties and Responsibilities:
- To network with other partners in ILTC settings to enhance continual support to patients and their families post-discharge from community hospitals, ensuring optimum health outcomes.
- To conduct post-discharge follow-up via phone calls and/or home visits to ensure patients and caregivers can cope with home care by helping streamline and coordinate available community services.
- To help patients and their families plan for and improve end-of-life care for those with terminal illnesses.
- To participate in hospital activities that contribute to improving patient care quality, such as research or innovative projects enhancing patient care.
- To be an advocate for patients and their families.
- To develop generic care management tools to coordinate care for patients.
- To identify, analyze, and address key problems in holistic patient management.
- Assist in preparing reports and statistics as required.
- Supervise junior staff.
Requirements:
- Professional qualification in Nursing, Social Work, or Allied Health, or any clinical discipline relevant to the job.
- At least 2 years of experience as a Care Coordinator in an acute or community setting in Singapore.
Estimated Salary: SGD $60,000 - SGD $80,000 per annum, depending on qualifications and experience.
