Job Summary
We are seeking a Care Coordinator for Elderly Services to join our team. The successful candidate will be responsible for coordinating the admission of residents under Ministry of Health (MOH) Portable Subsidy, providing financial counselling, and assisting with applications for government schemes.
Key Responsibilities
- Admission Coordination: Act as a coordinator for the admission of residents under MOH Portable Subsidy, ensuring seamless transitions and efficient processes.
- Financial Counselling: Perform financial counselling for all residents under MOH's Portable Subsidy, including preparation of estimated bill sizes, admission contracts, and administrative matters.
- National Means Testing: Perform and monitor National Means Testing (NMTS) for portable subsidy residents, ensuring valid and updated resident NMTS according to MOH's requirements.
- Resource Person: Act as a resource person for the MOH ILTC portal and IRMs or all ECON Medicare Centres and Nursing Homes.
- Application Support: Assist and provide support to your direct supervisor in putting up applications for subsidy Nursing Home for ECON private residents (private to subsidy conversion).
- Government Scheme Applications: Assist home and centre in putting up the following applications for their residents: Shelter Home, Day Care, Day Rehab, Home Modification, Senior Mobility Fund, and other available schemes.
- Resident Support: Assist the resident in putting up applications for government schemes, including Interim Disability Assistance Programme for the Elderly (IDAPE), Eldershield Claim, Medical Fee Exemption Card (MFEC), Medifund Claim, Elderfund, Careshield Life/Medishield Life, Home Caregiver Grant, PGDAS, Silver Support Schemes Encashment, and other financial schemes.
- Process Improvement: Provide support to your direct supervisor in ensuring that all processes and workflows are customer-focused, efficient, and achieve the organization's objectives.
- Collaboration: Identify, develop, and implement plans for areas of improvement, working together with colleagues as a team within the department or from other departments in a collaborative manner.
- Additional Responsibilities: Perform such other duties and responsibilities that may be assigned from time to time.
Requirements:
- Qualification: Diploma in Counselling/Psychology or Social Services or any equivalent qualification with at least 1 year relevant working experience.
- Skills: Good interpersonal and communication skills, team player, willing to work with people with disability, ability to work independently, ability to multi-task, ability to work effectively in a team environment, possess planning, communication, and interpersonal skills, empathetic and compassionate, proficient with MS Office, ability to work under pressure and meet tight deadlines.
Estimated Salary: $50,000 - $60,000 per annum, depending on experience.
