Social Worker - Residential Care Manager

About the Role:

We are seeking a dedicated and experienced Social Worker to join our Residential Case Management Unit as a Residential Care Manager. This is an exciting opportunity for a skilled professional to make a positive impact in the lives of residents and contribute to the success of our team.

About You:

To be successful in this role, you will need to have a degree in Social Work and at least 1-2 years of casework and counseling experience. Strong care management and counseling skills are essential, along with excellent interpersonal and communication skills. Certification in mandatory counseling cases is advantageous.

Your Key Responsibilities:

• Develop and implement individual care plans for all residents
• Conduct clinical intervention components in the Centre, including counseling and caring services
• Assist in ensuring safe custody of residents
• Collaborate closely with other departments, government agencies, and healthcare institutions
• Monitor security measures to ensure adequacy in the Centre
• Submit reports on serious incidents and occurrences to the Home's Management Committee
• Work in close liaison with law enforcement, welfare regulators, and related agencies
• Assess potential clients prior to admission to ensure suitability
• Prepare timely reports for submission and vetting by the Deputy Head of Home
• Maintain accurate records, accounts, documents, and correspondence
• Facilitate applications and renewals of Medical Fee Exemption Cards and Means testing for all residents
• Conduct group counseling, family sessions, one-to-one sessions, and maintain case recordings
• Perform home/hospital visits and significant tasks as required
• Support the reintegration of residents and their families
• Present cases to staff members for updates
• Update residents' families on significant incidents/hospitalization/happenings
• Contribute to the development of the Centre and its operations
• Carry out assigned tasks by the Head of the Home, Licensee, and Governing Board
• Lodge police reports when necessary
