Palliative Care Specialist
- Detailed Job Summary:
We are seeking a compassionate Palliative Care Specialist to join our interdisciplinary team. The ideal candidate will possess exceptional communication skills, empathy, and the ability to work collaboratively with patients, families, and healthcare professionals.
- Responsibilities:
- Facilitate socio-economic evaluations to address practical concerns for patients and their families, ensuring they receive optimal end-of-life care.
- Provide individualized counseling sessions through face-to-face interactions or home visitations to navigate personal, ethical, cultural, and spiritual dilemmas.
- Cultivate partnerships with local organizations to support patients in achieving their end-of-life goals.
- Empower family involvement, encourage visits, and prepare patients/families for the end-of-life stage, including life closure and the dying process.
- Offer bereavement education and support to help families cope with loss and grief.
- Participate in Interdisciplinary Patient Care Conferences to discuss comprehensive patient management plans.
- Advise patients and their families on financial and discharge care planning.
- Engage in fieldwork, visiting patients in hospital wards and social agencies, to alleviate or resolve medico-social problems.
- Prepare reports for management and relevant institutions/agencies.
- Present cases for discussion and actively engage in departmental or inter-departmental meetings to share, review, and clarify complex issues.
- Maintain continuous networking with MSW, Counselors, staff nurses, nursing officers, and medical staff/authorities of other organizations.
- Take on additional workload or difficult cases when required.
- Orientate and train new staff/interns on their duties.
- Participate in in-service education programs and ensure the transfer of learning.
- Present at Journal club the works and development in palliative and end-of-life care settings.
- Document all social interventions within the expected timeframe set by the department.
